Is it the intent of section 3007.6.4 that if an enclosed fire service access elevator lobby is provided at the level of exit discharge it must meet the minimum size requirement of 150 square feet

Yes, Although, Section 3007.6.2 gives an exception where enclosed FSAE lobby would not be required at the levels of exit discharge, if an enclosure is built - it needs to meet the requirements of 3007.6.4.
