Is it the intent of Exception 4 to require sheet metal from the air handler to the diffuser/grill?

Yes. The designer must take into account the need for all
metal to save the expense of the damper.

Commentary
The section is clear that the sheet steel duct must be
continuous between the air outlet and inlet terminals.
