Is it the intent of R311.5.3 that all dimensions and dimensioned surfaces be inclusive of carpets, rugs or runners

No. The section specifically states the dimensions
are EXCLUSIVE of carpets, rugs, and runners.

The thickness of the carpet, rug, or runner is not
standardized and could make code legal dimensions appear to
not meet the code requirement.
