Question
Is it the intent that the exceptions allow the F and T rating indicated in 712.4.1.1.2 to be eliminated if penetrating item is 6" maximum diameter.

No. The Exception allows the elimination of the T-rating where
the floor penetrations are contained and located
within the cavity of a wall. The 6-inch max. provision relates
to the limit of the penetration to a single fire-resistance
rated floor assembly as specified in Section 712.4.1.1
